Menopause, nutrition and bone loss: what dietitians need to know

The years around the final period are when a woman loses bone fastest. Most of the protective levers, calcium, vitamin D, protein and loading, sit squarely in your remit.

The menopause transition is the fastest period of bone loss in a woman's life, and much of what protects her, adequate calcium and vitamin D, enough protein, and the right kind of exercise, is dietetic. Knowing the mechanism, the targets and the referral lines lets you protect bone in the window that matters most, rather than reacting to a fracture years later.

Why the transition is the danger window

Food-first is the rule: dairy, fortified alternatives, tinned fish with bones, tofu set with calcium, and leafy greens. Supplements fill a gap; they are not a default. The nuance worth teaching patients is that more is not better, intakes pushed well above target add no further bone benefit and raise cardiovascular questions, so the dietetic skill is hitting the number through diet, then topping up only the shortfall.

Vitamin D, and the co-factors

Vitamin D of roughly 800 to 2000 IU a day, titrated to status, supports calcium absorption and muscle function. Whole soy foods belong in a varied diet; isolated high-dose supplements should not be presented as proven bone protection. Being straight about this protects both the patient and your credibility, and keeps the focus on the levers that work.
